The EH&D department are here to support our older Indigenous community to remain active & live independently within their homes as long as possible. We offer services through the following programs: ‘Home and Community Care’ (HACC) and ‘Commonwealth Home Support Program’ (CHSP) for Aboriginal people between the ages of 45-50 years. For Aboriginal people over the age of 50 years with more complex care needs the ‘Home Care Package’ (Levels 1-4) is available.
Home & Community Care (HACC) and Commonwealth Home Support Program (CHSP) offers:
- Support to participate in social activities in a group or one-on-one (planned activity group)
- Assistance with everyday household tasks (domestic assistance)
- Assistance to enhance nutrition & meal preparation through services (such as meals on wheels)
- Assistance to support your independence in your personal care activities such as showering, dressing, shopping, and attending appointments
- Assistance to keep yards hazard free by lawn mowing and rubbish removal
To be eligible for CHSP/ HACC:
- Eligibility is based on your support needs and age
- Be living in the community and be between the ages of 45-50 years old
- Individuals will need to be assessed for eligibility after applying

Home Care Package Level 1-4:
Each package level caters for different individual care needs. Your case manager will connect you to services that meets your individual goals and care needs to support your health and wellbeing, and to keep you living safely and independently at home.
Some examples of services provided through HCP Levels 1-4 include:
- Personal Care, Domestic Care, Transport
- Nursing, Respite & Allied Health Therapy
- Nutrition, meal preparation & diet advice
- Continence Management
- Supports for mobility & dexterity
- Home & Lawn maintenance
- Skin integrity management
- Support for cognitive impairment
- Social support group

To be eligible for a Home Care Package the individual needs to be:
- Assessed by the Aged Care Assessment (ACAS) team as to having complex care needs
- Notice a change in memory or cognition
- Diagnosed with a medical condition or reduced mobility
- Experienced a change in family care arrangements
- Experienced a recent fall or hospital admission
