Care Forward is a locally owned and operated organisation. We’ve been empowering Tasmanians to live well at home since 1999. Since its inception Care Forward has continued to grow in size, reach and service. We have a truly unique, innovative model providing independent clinically led care coordination.
Our dedicated and passionate team includes nurses, allied health professionals and home independence support staff – all working to lead our service provision, information and coordination, with the aim of maximising each persons’ independence, wellbeing and confidence at home. We don’t just provide and coordinate care and services, we empower thousands of Tasmanians across the State to take control of their own health and wellbeing by involving individuals at the centre of decision-making.
Care is coordinated across Tasmania with a continued commitment to moving your health and home care forward, specialising in health and homecare services designed to support the independence of older Tasmanians and people living with chronic conditions or disabilities.
We have local teams, with offices and Wellness Centres in Hobart, Launceston, Devonport and Burnie and will continue expanding and diversifying our in-home services and increasing the range of services available in our Wellness Centres led by our team of health professionals.
Our in house services, coordinators and health professionals are complemented by over 80 other subcontracted service providers, businesses and individuals. This enables us to provide you with access to a variety of service types across Tasmania.

Wellness Centres
Individually tailored strength and balance programs in a safe, inclusive, fun and supported environment increasing your mobility, independence and confidence as well as group education sessions.
Independence Programs
Care Forward specialises in reablement services for people with frailty, chronic illness or following a health setback who wish to make gains in their physical, social and emotional wellbeing. This may be a single service or an intensive individualised package of supports for up to 6 weeks, focusing on regaining lost functionality and/or learning new skills to ensure that you continue to live at home independently.

Care Forward aims to maximise benefits and minimise client fees across all programs. Some programs will require a small client contribution that is used directly on your services. Options are always available if you are experiencing financial hardship.
