Coolibah Care’s 136-bed residential facility is located in the heart of Mandurah. Every room has a private ensuite, built-in wardrobe and television, and can be personalised to feel like home.

Residential care at Coolibah Care covers a full range of needs, from medium and high-level care through to dementia-specific and palliative care. Registered nursing is available around the clock, and on-site specialists include clinical nurse practitioners, occupational therapists, dietitians and a podiatrist.

Families are partners in the care Coolibah Care provides. Regular care reviews, open communication and a team that takes the time to know each resident and their history are central to the Coolibah Care approach.

Specialised Dementia Care and Palliative Care

Coolibah Care has two dedicated dementia care areas, purpose-designed for people living with dementia. Both are secure environments where residents can move about freely and safely, with access to gardens and shared spaces. Care is delivered by a team trained in dementia-specific practice, with consistent routines and person-centred planning at the centre of everything.

Palliative care is provided by a specialist team with the aim of supporting comfort, dignity and quality of life in the final stages of illness. Families are closely supported throughout, and care is guided by each person’s wishes and values.

Support at Home

For older Mandurah residents who want to remain living independently, Coolibah Care’s Support at Home service provides practical assistance at home and in the community. Services are tailored to individual needs and can include personal care, domestic support, social activities and transport.

Government-funded Support at Home services are accessed through an aged care assessment via My Aged Care, or Coolibah Care can provide private services to suit individual needs. The team is happy to help clients and families understand their options and work out the right next step.
