In order to receive a funded Home Care Package, you need to be referred to My Aged Care and assessed by the Regional Assessment Service (RAS) in your area. You also need to be Income-tested by Centrelink in order for your own contribution to be determined.
If you are assessed as eligible for a package, you will be put in touch with Home Care Package providers in your area, or you can contact PRONIA directly.
Once you have been offered a package by a Home Care provider, they will help you plan your package based on your specific needs.
The maximum fee chargeable by a provider for a Home Care Package is set by the Department of Social Services. For a person on a single pension the fee may not exceed 17.5% of that pension. Fee contributions are discussed with each person in accordance with Department guidelines and are considerate of the personās individual circumstances.
Consumer Directed Care
The Home Care Packages at PRONIA are now delivered on a Consumer Directed Care model. The packages have been designed to support you to live independently in your own home. PRONIA will support you to manage your package with as much or as little support you require. You will determine what level of self-direction suits you.
PRONIA will sit down with you and your family/friend (if requested) and discuss the things that most matter in your life. You will be supported in creating your individual care plan. The care plan will reflect your current needs and future aspirational goals. The care plan will be tailor made to meet your individual care needs and can be flexible enough to change as your care needs change.
As part of the new consumer directed care model you will determine which services will provide care in your home and how much of your budget you will allocate to your care needs. You will be able to plan current and future needs with greater control and flexibility.
Types of service in Home Care Packages
The types of services provided under a Home Care Package will depend on your needs.
Services may include:
- Personal care ā such as help with showering or bathing, dressing, mobility, meal preparation and eating
- Support services ā such as help with washing and ironing, house cleaning and transport to help you do shopping, visit your doctor or attend social activities
- clinical care ā such as nursing and other allied health support services
