We work alongside approved aged care providers, including Trilogy Care, to help eligible clients access services through the My Aged Care Support at Home Program, including the End of Life Pathway, which provides up to $25,000 in additional funding for eligible people with rapidly changing end-of-life care needs.

Our Registered Nurses, Enrolled Nurses and dedicated care team work closely with GPs, specialists and families to provide expert clinical care tailored to each person’s individual needs.

Sisternightingale Dedicated Palliative and End-of-Life Care

Sisternightingale Holistic Personal Care and Lifestyle Support
Exceptional care extends beyond clinical treatment. We support people to maintain their independence, dignity and quality of life through personalised assistance with everyday living.
Our services include:
- Personal care, showering, dressing and grooming
- Meal preparation and nutritional support
- Dementia and cognitive support
- Domestic assistance and household tasks
- Transport to medical appointments and community outings
- Coordination with physiotherapists, occupational therapists, dietitians, podiatrists and other allied health professionals
Every care plan is tailored to the individual, ensuring support is flexible, respectful and responsive as needs change.
Navigating the Support at Home Program and Funding
Understanding aged care funding can feel overwhelming, particularly during times of illness or when care needs are changing. At Sisternightingale, we help families understand their options and navigate the funding process with confidence.
While we are not currently an approved Support at Home provider, we work in partnership with approved providers, including Trilogy Care, allowing eligible clients to access Sisternightingale’s nursing services through their government-funded care package where appropriate.
For eligible people aged 65 years and over who are approaching the end of life, we also assist families to access the My Aged Care End of Life Pathway, which provides up to $25,000 in additional funding over approximately four months for eligible individuals with rapidly changing care needs.
