Navigating the process

Our expert, friendly home care team can assist you to apply for a home care package, how to change your current provider and how to navigate the process. We’ll work with you to develop a personalised plan of care that meets your needs, arrange assessments and provide care options to support you to live independently. You will have direct contact with a case manager and regular face-to-face visits to review your care and support.
